Double Sequential Defibrillation Framing
Objective: Keep DSED/vector-change in the “only if trained and authorized after multiple failed shocks” box — never instead of CPR and the first defibrillation.
Why This Is Hard
Refractory VF videos make dual-sequence look like magic. AHA 2025 ALS: after three or more consecutive shocks, the usefulness of double sequential defibrillation or vector change has not been established (Class 2b). It is not first-line.
- You probably have one defibrillator. Vector-change (pad move) may be the realistic option if protocol includes it.
- Do not steal a second monitor from another crew as theater if it delays CPR.
- This site does not teach a pad diagram as an order. Follow your medical director.
Field Rules (Education)
- First: early defibrillation and high-quality CPR. That is still the survival intervention.
- Class 2b: usefulness of DSED or vector-change after ≥3 consecutive shocks has not been established — not instead of epinephrine timing, an airway, or reversible causes.
- If you only have one device: vector-change (new pad position) is what most EMS protocols mean, if they mean anything.
- Fine VF is still shockable. Do not “need DSED” because you under-gained asystole.
Field Pitfalls
- Delaying the first shock to set up two monitors.
- Calling DSED standard of care.
- Skipping CPR to film the dual shock.
